This is a third-party administrator plan which means that it is a family of different insurance companies. Your specific insurance company name and customer service number is on the front and/or back of your insurance card. To find out what type of vision coverage you have, you will need to contact them.

Please note that most First Health plans are PPOs, which means you would not need a referral for a medical visit at our office (i.e., pink eye, scratched cornea, etc).
